  • Breast reconstruction is a procedure to rebuild a breast which has been completely or partly removed. The most common reason for undergoing breast reconstruction surgery is for patients who have had breast cancer surgery (mastectomy or lumpectomy). In the US, breast cancer affects around 1 in 8 women.

The surgery is often performed at the same time as a mastectomy, however some patients may choose to wait a while before undergoing a breast reconstruction. Patients who are undergoing radiation therapy are advised to undergo breast reconstruction in stages, as they cannot undergo reconstruction whilst receiving radiation therapy, as this could damage the new breast.

There breast can be reconstructed either with breast implants or with flaps. Implants are made of silicon or saline and are an inserted under the chest muscle. Flaps are made up of tissue which is taken from another area of the body such as the buttocks or thighs, and are connected to the blood vessels on the chest. With a flap procedure, the shape and size of the breast will change in the first few months and once it has formed it's shape, nipple reconstruction may then be performed. Nipple reconstruction involves taking part of the nipple from the other breast and filling in the color by tattooing it to the nipple.

As there are a number of different surgical options for patients undergoing breast reconstruction, the time needed for the treatments vary considerably. As a guideline:

    • Mastectomy with placement of a tissue expander - 1 to 2 weeks
    • Mastectomy with latissimus dorsi flap - 2 to 3 weeks
    • Mastectomy with pedicle TRAM flap - 6 to 8 weeks
    • Mastectomy with abdominal free flap (free TRAM, DIEP or SIEA flap) - 4 to 6 weeks
    • Mastectomy with GAP flap - 6 to 8 weeks
    • Exchange tissue expander to implant - 1 week or less
    • Exchange unilateral tissue expander and augment/reduce/lift opposite breast - 1 to 2 weeks
    • Nipple areola reconstruction - 1 week or less

The 1st floor of the clinic was not what you'd typically expect for a hospital or Surgery center. But once you go up to the 2nd and 3rd floors where the beds and operations are, everything was much more modern and familiar.

The staff was very straightforward about what they were doing and what the price was. I was easily able to pay with my credit card and get my surgery immediately afterwards. The surgery was just as good as any surgery I've had, and the anesthesiologist was better than anyone I've had! The whole operation went smoothly and recovery is happening very quickly. They provided me with a prescription for pain medicine and antibiotics which I was able to pick up at their pharmacy across the street for about $20.

I brought my friend as my personal interpreter, but they did have people on staff who spoke very fluent English as well. it was easy to coordinate everything with my surgeon's patient coordinator through Whatsapp

Dr. Victor Mendoza is a very skilled surgeon. I was more than pleased with the results of my surgery. It far exceeded my previous surgeries performed in the states. I found Dr. Mendoza to be a very warm and engaging, sensitive to my needs and very informative about all issues regarding my surgery. While recovering after surgery the staff was very attentive and caring making sure I was as comfortable as possible. Dr. Mendoza was very good about checking up with any concerns and needs after surgery. We had some issues with transportation in the beginning but they were resolved shortly. If you are considering cosmetic surgery I don't think you could find a better clinic or doctor on either side of the border!

I had a transvaginal hysterectomy performed, secondary to my endometrium developing tumors that penetrated my uterus. The tumors developed following exposure to the breast cancer drug tamoxifen. Here in the US, my doctor stated he was willing to perform a hysterectomy, but only if I allowed him to cut me open and use a laparoscope. The last time they did this, I herniated at the lap sites, which necessitated further surgery, and I was unwilling to have this done this way as it would almost certainly happen again.

So I searched for a facility that would be willing to perform the surgery transvaginally, which is how the majority of hysterectomies are now performed. Vaginal hysterectomies have no herniation concerns, and almost no infection risk, super fast healing times, and remarkably few complications. Salutaris stated their capacity to perform this was dependent upon my anatomy, so I had to pay for copies of the CT and ultrasound, and then painfully assembled a composite image in all axes of the approximately thousand separate images those contained. All together, this was not only costly, it took about 30 hours of work on my end. Upon reviewing the data, Salutaris agreed to the procedure.

But when I arrived, they insisted on redoing the ultrasound (at an extra cost, no less), then declared that my vagina was too small for the procedure and they'd have to cut me open, like a c-section. I was already in Guadalajara at this point, and had been cleared through FMLA at work, and wouldn't be allocated additional time if I refused the surgery. I argued with them pretty intensely, and one of the things I pointed out was that they'd already agreed to the transvaginal procedure on the basis of the scan images I sent. They then denied ever receiving the images of the scans.

At the end of all of it, what we agreed to was that the surgeon would perform the vaginal hysterectomy if, after receiving anesthesia, he was able to dilate me sufficiently to perform it. Between the anesthetic and muscle relaxant, they were able to accomplish this.

Here are some positives: the room I recovered in was massive and what I'd expect to find at a spa. The bathroom was beautiful, there was a full-sized leather couch, leather chair, and coffee table in the "living room" section of the room, and they provided a little "care kit" containing a toothbrush, toothpaste, shampoo, lotion, and slippers for free. Additionally, they had wifi, and the windows were so large it was naturally illuminated and very cheery. Food was boring, but fine. I think they deliberately had me on a restricted diet for after care; what was given was tasty enough. There was a tv, but the shows were all in Spanish (of course) and not subtitled.

The anesthesia and pain management was simply superb. This was a real concern for me as narcotics traditionally don't work on me, but they thankfully used a more intelligent, less risky approach with intrathecal nerve block, and alternating doses of IV acetaminophen (Tylenol) and IV diclofenac, so I had an epidural catheter in my spine until I was discharged in addition to an IV.

Communication was occasionally iffy. The surgeon didn't speak English, but provided a translator at no cost for our initial visit, and came with the anesthesiologist every other time, who was perfectly fluent. The hospitalists (physicians rounding on patients during their stay) all were perfectly fluent. None of the nurses spoke any English, and this was an issue as my IV kept clotting off, and I kept calling for help, but they only occasionally came, and didn't understand what I was asking. When they'd understand, they'd force the clot, which eventually led to my vein blowing, but even that, though painful and still palpable to this day, was ok as I was only there for a day, and the solutions they were tranfusing weren't caustic. I used the wifi to translate simple phrases, and we were mostly ok with that.

Ultimately, my highest concerns were that they perform the surgery correctly, and that my pain would be managed. They did such a good job with both that I was walking within three hours of the surgery ending, and my pain was 0/10 on the pain scale for almost the entire day I was in their care, only raising once to about a 4/10 when nursing had forgotten a dose of the nerve block. The hospital sent me out with a prescription for tylenol, which wasn't sufficient, but pain was still shockingly low, and ibuprofen I bought at the pharmacy was sufficient to cover all my pain. Within four days, I was walking ten miles a day (I didn't have transportation there). It likely helped on all fronts that I'm somewhat athletic to begin with, and have a very physical job that requires a lot of walking and lifting (I do hospital work).

If you're going there for surgery, I'd recommend you bring ibuprofen with you, and a heating pad (with an outlet adaptor), even if it's only for after your discharge. These were both necessary for me. The outlet adaptor was needed even in the hospital.

We speak very little Spanish, so there were sometimes when the front desk or a nurse had to get someone who spoke English to clarify for us. Ofelia was our coordinator. She was a great help, spoke English quite well, was very competent, and always greeted us with a smile. My surgeon, Dr. Marco Marroquin Batres, was professional, reassuring, and explained everything completely. He encouraged questions and post surgery had us keep in touch with him for any questions using "What's App". I am very pleased with the surgery. My husband was so impressed with him that he plans to go back next year for eyelid surgery!

The clinic is located on a popular avenue so it's easy to get there. The interior is nice and the ambiance is warmer than any other clinics I ever have been to. The waiting room has direct light shining in and there are orange globes hanging from the celling, which makes the place stand out. The staff were really kind and supportive. They assisted with anything I requested with a smile on their faces.
